How we deal with independent safeguarding investigations
Every investigation begins with an initial discussion to understand the concern, the safeguarding context, the level of risk and the outcome required. We then agree the scope of the investigation and clarify the issues that need to be examined.
Once the scope is agreed, we review relevant records, policies, case notes, communications and any other documentation connected to the matter. Where needed, we build a clear chronology to identify events, decisions and key points of concern.
We then carry out a structured fact-finding process. This may include interviews, evidence review, analysis of documentation and consideration of the actions taken by those involved. Our approach is impartial, proportionate and focused on establishing the facts fairly.
At the end of the process, we produce a clear written report. Our reports set out the background, the scope of the investigation, the evidence considered, the analysis, the findings and any recommendations. This gives providers a robust written record and a clear basis for next steps.
